Make Up Final Exam Request Form - Spring 2024

OUE Accommodated Testing Services will proctor Spring 2024 makeup final exams.

This form is to be submitted by faculty or department staff only. Once we have received your request, we will notify the student of the exam time and location.

Exam requests should be made at least 1 business week in advance.
Exam(s) must be received by OUE at least 48 business hours prior to the exam date/time.

This form is intended for students with verified scheduling conflicts. If you have a student with accommodations from DAS, use the Final Exam Accommodation Request Form.

Section 2: Exam Instructions

Please email the exam to oue.testing@emory.edu at least 48 hours in advance of the exam date.

How will the exam be provided to the student?

I will send the paper exam to oue.testing@emory.edu.

Student will take the exam in-person using a personal computer. (If applicable - please share any access instructions/exam access code under the "specific instructions" prompt below)

What materials may a student bring to or use during the exam? (If none, please type N/A)

Are there any specific instructions for the exam the proctor team should be aware of? (We will assume 2.5 hours for the final exam unless otherwise notified)

Section 3: Exam Return

Exam Return: Completed exams will be scanned and returned to you electronically at the e-mail address you have listed. If you prefer the paper exam, you can pick them up from the OUE front desk in White Hall 220 during regular business hours.
