OUE Accommodated Testing Services will proctor exams for students who have been approved for exam accommodations through the Department of Accessibility Services (DAS). Students are requested to provide accommodation letters to their instructors for confirmation.

Up to three students with DAS accommodations in a single class: Fill out all sections of this form.

More than three students with DAS accommodations in a single class: Fill out sections 1 - 3 of this form, download this spreadsheet, insert student info, and email the completed file to oue.testing@emory.edu.

Exam requests should be made at least 1 business week in advance.
Exam(s) must be received at least 48 business hours prior to the exam date/time.

How will the exam be provided to the student?

I will send the paper exam to oue.testing@emory.edu or hand deliver to White Hall 220.

Student will take the exam in-person using a personal computer

Exams must be received by OUE at least 48 hours prior to the exam date/time. Please include the student name and ID number on the email or envelope if delivering to WH 220.

What is the original exam date for all students in your course?

OUE ACTS can typically offer Monday-Thursday evening sessions, Wednesday afternoon sessions; and, all day Friday. We always aim to schedule exams in closest proximity possible to the original time.

Exam Day and Start Time: OUE will schedule exams for the earliest available session in proximity to the original exam date/time. If you or the student have a time preference, please indicate so below. Otherwise, please leave blank. Once received, the OUE Testing Coordinator will email you and the student with exam details. All re-scheduling requests should go through oue.testing@emory.edu. OUE will only reschedule exams with the instructor's permission.
