MakeUp Final Exam Request Form

***This form is for MAKEUP FINAL EXAMS in Emory College courses only***

A Makeup Final Exam is an exam rescheduled due to having three or more exams in one calendar day or two overlapping exams scheduled during the same time period. Students must submit the Three Exams in One Calendar Day form and OUE will verify the conflict.

Faculty should only submit this form after receiving OUE email notification of verification of the conflict, and after discussing alternatives with the student. Please determine the appropriate date for the makeup exam with the student, and indicate it on the form below.

IMPORTANT NOTES:

This form must be submitted by Wednesday, April 22 at 11:59pm.
Final Exam(s) must be delivered to OUE at least 48 business hours prior to the exam date/time. OUE ACTS cannot proctor an exam without timely submission.
This form is not intended for students with testing accommodations. If you have a student with accommodations from DAS, use the Exam Accommodation Request Form.

Contact Information: Should the proctor need to contact you during the exam session with a time-sensitive question (e.g., digital exam access code issues), please provide an immediate contact method, reachable during non-business hours. If you provide a phone number, please indicate your contact method preference (i.e., call or text, or both).

Any information provided will not be directly shared with students. It is recommended that instructors monitor their communication for a few minutes after the exam session start time. If the proctor does not receive a response within 25 minutes, the student(s) will be rescheduled for another day.

Section 2: Exam Instructions

Exam must be delivered to OUE (either via email to oue.testing@emory.edu or in-person) at least 48 business hours prior to the exam date/time.

What is the format of the exam?

Paper Exam: I will deliver exam via email to oue.testing@emory.edu or drop off in-person to Candler Library 200 at least 48 business hours prior to the exam date/time.

Online Exam: Student will take the exam in-person using a personal computer. (If applicable - please share any access instructions/exam access code under the "specific instructions" prompt below.)

What materials may a student bring to or use during the exam? (If none, please type N/A)

Are there any specific instructions for the exam the proctor team should be aware of? (We will assume 2.5 hours for the final exam unless otherwise notified)

Please coordinate an appropriate date for the makeup exam with the student. After you have coordinated the date, please select the date below. The student will receive confirmation details at least one week before their final exam date.

Section 3: Exam Return

IMPORTANT NOTE:
Completed exams will be returned by the end of the next business day. For example, completed exams for a final exam held on Friday, December 12th will be returned by the end of day on Monday, December 15th.

Please select how you would like to receive the completed exam:

Paper exam – Scan and return via email

Paper exam – Instructor or TA will pick-up hard copies from Candler Library, Suite 200

Digital exam - No pick-up needed (OUE ACTS will hold onto any scratch paper which you may arrange to pick up if needed)

Note that we cannot scan blue books. Exams using blue books must be picked up from Candler Library 200.
