Exam Accommodation Request Form

OUE ACTS is an optional service, as instructors may choose to manage accommodations independently. This service is only available for students who have DAS testing accommodations.

Exam sessions will be unavailable on the following dates. You should make your own arrangements if your exam must be administered on one of these days.

Important Notes for Instructors:

Exam requests must be made at least one (1) week in advance.
Exams must be delivered to OUE (either via email or in-person) at least 72 business hours prior to the exam date/time.
Instructors must submit this form for each exam but may do so as far in advance as they choose.

Tip for Instructors:

Before filling out this form, you will need to know student details such as name, Emory email, and EMPLID. This information can be found on the student's accommodation letter or in your OPUS class roster.

What happens after I submit this form?

As soon as you submit this form, an automated email with a registration link will be sent to the student(s). You will be copied on this email. It is the responsibility of each student to follow the directions in the email which includes the acceptable date range for the exam.
You should follow up with students if they select an unacceptable date for their exam.
When a student registers for a session, an automated email confirmation will be sent to you and the student.

Contact Information: Should the proctor need to contact you during the exam session with a time-sensitive question (e.g., online exam access code issues), please provide an immediate contact method, reachable during non-business hours. If you provide a phone number, please indicate your contact method preference (i.e., call or text, or both).
Any information provided will not be directly shared with students. It is recommended that instructors monitor their communication for a few minutes after the exam session start time. If the proctor does not receive a response within 25 minutes, the student(s) will be rescheduled for another day.

Exam Instructions

How will the exam be provided to the student?

I will send the paper exam to oue.testing@emory.edu for printing

I will deliver printed exams to Candler Library, Suite 200

Student will take the exam in-person using a personal computer

For online exams, please provide the access code and any other pertinent instructions.

Exam must be delivered to OUE (either via email or in-person) at least 72 business hours prior to the exam date/time. All hand-delivered exams must be placed inside a sealed and labeled (instructor name, course) envelope.

What is the original exam date for all students in your course?

ATTENTION: This form must be submitted at least seven (7) days before the Original Exam Date. BEFORE YOU SUBMIT THIS FORM, please contact oue.testing@emory.edu to inquire about availability on your exam date.

What is the earliest date that a student can take this exam?

Keep in mind that students may have other classes or responsibilities that make it difficult for them to take an exam with extended time on the Original Exam Date. Providing a reasonable range is extremely helpful for students and for OUE ACTS.

What is the latest date that a student can take this exam?

What is the standard exam length? (OUE will calculate extended time based on student accommodations.)

What materials may a student bring to or use during the exam? (If none, please type N/A)

Pick-up from Candler Library 200

Scan and return via email

Digital exam: no return needed (OUE ACTS will hold onto any scrap paper for one semester; it will be available for pick up from CL 200 upon request.)

Note that we cannot scan blue books. Exams using blue books must be picked up from Candler Library 200.

IMPORTANT NOTE:
Completed exams will be returned by the end of the next business day. For example, completed exams for a final exam held on a Friday will be returned by the end of day on the following Monday.
