PHA LOR Hub Contact Form

Please complete this form if you are planning to use the PHA LOR Hub in a future cycle for delivery. PHA will e-mail instructions in the year indicated on this form.   

The LOR Hub is used to store and deliver: 1) Letter Packets or 2) previously completed Composite Letters.

1. Letter Packets- Alumni within 3 years of graduation are eligible to apply for a Letter Packet, which is comprised of 3-4 individual letters of recommendation. Individual letters of recommendation included in a Letter Packet must be requested and dated within the current application cycle. Letter Packets from previous application cycles cannot be reused; a new Letter Packet must be created for each cycle.
2. Existing Composite Letters - Alumni within 3 years of graduation with an existing Composite Letter on file with PHA may elect to have their Composite Letter delivered to health professional programs or Centralized Application Services by submitting a request through the PHA LOR Hub.
