PHA LOR Hub Contact Form

Please complete this form if you are planning to apply for the PHA LOR Hub in a future cycle. The LOR Hub service is used to store and deliver: 1) Letter Packets or 2) previously completed Composite Letters.

1. Letter Packets- Alumni within 2 years of graduation are eligible to apply for a Letter Packet, which is comprised of 3-4 individual letters of recommendation.
2. Existing Composite Letters - Alumni within 3 years of graduation with an existing Composite Letter on file with PHA may elect to have their Composite Letter delivered to health professional programs or Centralized Application Services by submitting a request through the PHA LOR Hub.

The LOR Hub application typically opens from May 1 to August 1 each year.
