OUE Accommodated Testing Services will proctor Fall 2024 makeup final exams.
This form is to be submitted by faculty or department staff only. Once we have received your request, we will notify the student of the exam time and location.
Exam requests must be made at least 1 business week in advance.
Exam(s) must be received by OUE at least 48 business hours prior to the exam date/time.
This form is intended for students with verified scheduling conflicts.
If you have a student with accommodations from DAS, use the Final Exam Accommodation Request Form.

Course Title
Should the proctor need to contact you during the exam session with an immediate question (e.g., digital exam access code issues), please provide an immediate contact method (reachable during non-business hours).
If number, please indicate contact method preference (i.e., call or text, or both).
Any information provided will not be directly shared with students.
It is recommended that instructors monitor their communication for a few minutes after the exam session start time.
If the proctor does not receive a response within 25 minutes, the student(s) will be rescheduled for another day.

Exam must be delivered to OUE (either via email or in-person) at least 48 business hours prior to the exam date/time.
All hand delivered exams must be placed inside a sealed and labeled (instructor name, course), manila envelope; and delivered to OUE at least 48 business hours prior to the exam date/time.

Section 3: Exam Return
Exam Return: Completed exams will be scanned and returned to you electronically at the e-mail address you have listed. If you prefer the paper exam, email oue.testing@emory.edu to coordinate a pick up from the OUE's front desk in Candler Library, Suite 200, during regular business hours.
